- Patient Intake Form: Provides us with your medical and mental health history. We will provide you with a link to access the patient portal.
- Consent to Treatment Form: Required for all new patients.
- HIPAA Privacy Acknowledgment Form: Protects your privacy and ensures compliance with federal law.
- We are an out-of-network (OON) practice, which means we do not directly bill insurance companies. However, many insurance plans offer 50% to 80% reimbursement for out-of-network mental health services.
- After your visit, we will provide you with a superbill (an itemized receipt) that you can submit to your insurance provider for reimbursement.
